Wormholes. We tend to think of them in terms of the movie/TV show Stargate. Those kind are possible, and have been achieved, but the more common type is the microscopic kind. The ones that exist between all atomic particles in the universe. Slightly less common are the ones seated in your brainpan.
This miniature stargate is the link between your squishy gray matter and the "upper levels", for lack of a better term.
This tiny wormhole opens up just enough for a stable two way data-stream to be possible.
Though some people have learned to 'dilate' theirs, to atone for a greater transfer of data (including matter – their whole bodies) through the portal.
Yes, that's teleportation.
